Tiludronate Dosing

For the treatment of Paget's disease, most people start with a tiludronate dose of two tablets once a day for three months. If needed, your healthcare provider may recommend re-treatment after the first three months. To help prevent negative interactions with foods, beverages, or other drugs, make sure to follow tiludronate dosing guidelines carefully.

Tiludronate Dosage: An Introduction

There is only one standard dose of tiludronate disodium (Skelid®), regardless of your age, weight, or other medical conditions. As is always the case, do not adjust your tiludronate dose unless your healthcare provider specifically instructs you to do so.
 

Tiludronate Dosing for Paget's Disease

According to the manufacturer of tiludronate, the recommended starting tiludronate dose for treating Paget's disease of bone is 400 mg once daily for three months. However, this is a little confusing, since tiludronate comes only in 240 mg tablets. Each tablet contains 240 mg of tiludronate disodium, which is equivalent to 200 mg of tiludronic acid.
 
It is assumed that the manufacturer's recommended dosage is referring to the acid form (not the disodium form), although this is not the standard practice. Therefore, the recommended dose is two tablets of tiludronate (480 mg of tiludronate disodium, which is equivalent to 400 mg of tiludronic acid) once daily for three months.
 
If necessary, you may be re-treated with tiludronate after at least three months after the initial treatment.
 

General Information on Dosing With Tiludronate

Considerations for people taking tiludronate include the following:
 
  • Tiludronate comes in tablet form. It is taken by mouth every day for three months
     
  • Take tiludronate on an empty stomach first thing in the morning. Do not lie down for at least 30 minutes after taking a dose. Do not eat for two hours before or after taking tiludronate. Many foods interact with tiludronate, decreasing its absorption into the body (making it less effective).
     
  • Swallow tiludronate tablets whole with a full glass of plain water (not any other beverage).
     
  • Do not take vitamins, minerals, or antacids that contain calcium, iron, magnesium, or aluminum within two hours of taking tiludronate (see Drug Interactions With Tiludronate).
     
  • You need to make sure you get enough calcium and vitamin D while taking tiludronate. Tiludronate cannot adequately build bone without calcium and vitamin D, and calcium and vitamin D help to prevent low blood calcium caused by tiludronate.
     
  • It is best to take tiludronate at the same time each day to maintain an even level of the medication in your blood.
     
  • For the medication to work properly, it must be taken as prescribed.
     
  • If you are unsure about anything related to tiludronate or your tiludronate dosage, please talk with your doctor, nurse, or pharmacist. Do not stop taking the drug without first discussing it with your healthcare provider.
