Sandimmune Dosage

Dosing for Sandimmune Intravenous Injection

If you are unable to take Sandimmune by mouth before or after your surgery, you may be given it as an IV injection. The usual initial dosage in this case is 5 to 6 mg per kg (2.3 to 2.7 mg per pound) given 4 to 12 hours before transplantation. This amount will be continued until you are able to swallow Sandimmune capsules or oral solution.

General Information on Taking Sandimmune

Some considerations to keep in mind during treatment with Sandimmune include the following:
  • This medication comes in the form of a capsule, a liquid oral solution, and an injection. The capsules and oral solution are given by mouth once a day. The injection is given intravenously (into a vein, or by IV).
  • You can take Sandimmune dosages with food or on an empty stomach; however, you should take it in a consistent manner, with the same amount of time between doses and meals each day.
  • Sandimmune contains cyclosporine, a medication that is also available in a "modified" form. Modified cyclosporine has been slightly altered from regular cyclosporine and is more easily absorbed into the body.

Sandimmune and modified cyclosporine are not interchangeable. If you need to switch from one type or brand of cyclosporine to another, only do so under your healthcare provider's supervision. He or she may need to adjust your dosage.

  • The oral solution will taste better if mixed with milk, chocolate milk, or orange juice, preferably at room temperature. When mixing your dosage, use a glass container (not plastic) and drink the mixture immediately (do not allow it to stand). After drinking, rinse the glass with a little more liquid to make sure you get your entire dose.
  • Always use the supplied dosing syringe to measure out your liquid dosage. Do not rinse the syringe before or after use -- doing so may change the accuracy of the syringe. If the syringe needs to be cleaned, make sure it is completely dry before using it again.
  • Sandimmune intravenous injection comes as a concentrated solution. It must be diluted before use. Once diluted, it is given as a slow infusion into a vein over two to six hours.
  • For the medication to work properly, it must be taken as prescribed. Do not stop taking this medicine, or change to another brand of cyclosporine, without first talking to your healthcare provider.
