For preventing infections during chemotherapy, the recommended filgrastim dosage is 5 mcg per kg of weight daily. To prevent infections with bone marrow transplants, the suggested daily dose is 10 mcg per kg. To aid in stem cell collection, 10 mcg per kg is taken daily four days before the collection process. Filgrastim dosing for congenital neutropenia usually starts at 6 mcg per kg twice daily.
The dose of filgrastim (Neupogen®) that your healthcare provider recommends will vary depending on a number of factors, including:
- Your weight
- How you respond to filgrastim
- The condition being treated
- Other medical conditions you may have
- Other medications you may currently be taking.
As always, do not adjust your filgrastim dose unless your healthcare provider specifically instructs you to do so.
The recommended starting filgrastim dose for preventing infections during chemotherapy is based on body weight -- filgrastim 5 mcg per kg each day (about 2.3 mcg per pound daily) given either by an injection under the skin or by an IV. It should be started at least 24 hours after chemotherapy and continued for up to two weeks or until your neutrophil levels increase enough. If necessary, your healthcare provider may increase your filgrastim dose for subsequent chemotherapy cycles.
For preventing infections with bone marrow transplants, the recommend dose of filgrastim is 10 mcg per kg daily (about 4.5 mcg per pound each day) given by IV. The first dose should be given at least 24 hours after the transplantation. Your healthcare provider will adjust your dose based on your neutrophil levels and will stop the filgrastim when the neutrophil level stays high enough for three or more days.
