Fondaparinux Dosage

If you are treating or preventing blood clots with fondaparinux, the amount of your dosage will be based on several factors, such as your weight, the reason you are taking this medication, and other medical conditions you may have. This prescription blood thinner comes in the form of an injection, which is administered just under the skin once a day.

An Introduction to Dosing With Fondaparinux

The dose of fondaparinux sodium (Arixtra®) your healthcare provider recommends will vary, depending on a number of factors, including:
  • The reason you are taking fondaparinux
  • Your weight
  • Other medical conditions you may have.
As is always the case, do not adjust your dosage unless your healthcare provider specifically instructs you to do so.

Fondaparinux Dosage for Preventing Deep Vein Thrombosis (DVT) Clots

The standard dose of fondaparinux for preventing blood clots in people undergoing abdominal (stomach) surgery, hip replacement surgery, knee replacement surgery, or hip fracture surgery is fondaparinux 2.5 mg once daily. The first dose should be started no sooner than six to eight hours after surgery (starting it sooner increases the risk of bleeding). The typical length of treatment is five to nine days, although much longer courses (such as 24 days of treatment) are often recommended for people who have had hip fracture surgery.

Fondaparinux Dosage for Treating DVT and Pulmonary Embolism (PE) Clots

The standard dose of fondaparinux for treating DVT or PE depends on weight, as follows:
  • Less than 100 pounds -- fondaparinux 5 mg once daily
  • 110 to 220 pounds -- fondaparinux 7.5 mg once daily
  • Over 220 pounds -- fondaparinux 10 mg once daily.
For DVT or PE treatment, most people will also be started on warfarin (Coumadin®, Jantoven®), as warfarin is usually preferred as the long-term treatment. Fondaparinux should be continued for at least five days after the warfarin is started and only after laboratory tests show that the warfarin is effectively working.
