Dalteparin Dosage

Dalteparin Dosing for Treating Clots in People With Cancer

The standard dosage of dalteparin for the extended treatment of clotting in people with cancer varies, depending on body weight. Typically a higher dosage is used for the first month of treatment, as follows:
 
  • Less than 124 pounds -- 10,000 IU once daily for the first month, followed by 7500 IU once daily
  • 125 to 150 pounds -- 12,500 IU once daily for the first month, followed by 10,000 IU once daily
  • 151 to 181 pounds -- 15,000 IU once daily for the first month, followed by 12,500 IU once daily
  • 182 to 216 pounds -- 18,000 IU once daily for the first month, followed by 15,000 IU once daily
  • 217 pounds or more -- 18,000 IU once daily for the first month, followed by 18,000 IU once daily.
     

Dalteparin Dosing for "Cardiac Events"

The standard dalteparin dosage after an episode of unstable angina (a type of chest pain that may signal that a heart attack is near) or a non-Q-wave myocardial infarction (a type of mild heart attack) is 120 IU per kg (about 55 IU per pound) every 12 hours, up to a maximum of 10,000 IU every 12 hours.
 
In this situation, dalteparin is usually combined with aspirin (75 mg to 165 mg once daily). Dalteparin should be given until the person is stabilized (typically, treatment is continued for a total of 5 to 8 days).
 

Dalteparin Dosing for Other Situations

If you have kidney disease or low blood platelets, your healthcare provider may recommend a lower-than-standard dalteparin dosage. If you are taking dalteparin for an unapproved use (an "off-label" use), you should be aware that there are generally no clearly established dosages for such uses.
 

General Information on Using Dalteparin

Some considerations for people taking dalteparin include the following:
 
  • This medication is given as an injection just under the skin (as a subcutaneous injection) once or twice a day.
     
  • The injections are usually given in the fatty layer on the abdomen (the "stomach" area), the upper thigh, or the upper area of a buttock.
     
  • If you are in a hospital, a healthcare provider will probably administer the injection. However, you may be sent home with this medication, in which case you or a caregiver must administer the injections.
     
  • Make sure your healthcare provider shows you exactly how to inject dalteparin.
     
  • Do not inject this medication into a muscle. It should be injected into the fat layer under the skin.
     
  • For this drug to work properly, it must be used as prescribed. You may be increasing your risk for serious problems if you stop taking it too soon.
     
  • If you are unsure about anything related to your dosage of dalteparin, please talk with your healthcare provider, nurse, or pharmacist.
